Abstract

Hipertiroid adalah keadaan tirotoksikosis sebagai akibat dari produksi tiroid dimana kadar TSH serum <0,3µIU/ml dan FT4 >2 nano gram/dl. Diagnosa hipertiroid pada kehamilan ditegakkan melalui tes laboratorium karena tanda dan gejala sama dengan kehamilan pada umumnya. Hipertiroid kehamilan menjadi penyebab abortus pada trimester pertama kehamilan, meningkatkan risiko kelahiran prematur, IUGR, BBLR, pre-eklamsia, gagal jantung kongesif, dan IUFD. Penelitian ini bertujuan untuk mengetahui faktor risiko yang berhubungan dengan kejadian BBLR pada wanita hipertiroid di Kabupaten Magelang tahun 2014. Jenis penelitian adalah analitik observasional dengan pendekatan case control, dilengkapi dengan kajian kualitatif. Sampel penelitian adalah ibu hamil yang melahirkan bayi BBLR di tahun 2014 dan pernah mengikuti tes TSHs ibu hamil di laboratorium GAKI sebanyak 41 kasus dan 41 kontrol. Teknik pengambilan sampel secara Proportionate Stratified Random Sampling. Analisis data dilakukan secara univariat, bivariat, multivariat, dan analisis kualitatif. Hasil penelitian ini adalah faktor yang berhubungan dengan kejadian BBLR menurut analisis bivariat adalah LILA (p=0,023; OR= 0,695; 95% CI=0,263-1,833) , kenaikan BB (p=0,010; OR=0,078; 95%CI=0,009-0,638), dan status anemia (p=0,000; OR=0,126; 95% CI=0,046-0,344), sedangkan menurut analisis multivariat, adalah status hipertiroid kehamilan, kenaikan BB dan status anemia. Kesimpulan dari penelitian ini adalah Ibu hamil yang memiliki status hipertiroid kehamilan, kenaikan BB berisiko (<6kg atau >12 kg) dan memiliki status anemia (Hb<11gr%) akan memiliki probabilitas sebesar 89% untuk melahirkan bayi dengan berat badan rendah.. Diagnosis of hyperthyroidism in pregnancy enforced by laboratory tests for signs and symptoms are the same as with pregnancy in general. Hyperthyroidism pregnancy is the cause of abortion in the first trimester of pregnancy, increases the risk of preterm birth, IUGR, low birth weight, pre eclampsia, heart failure congestif, and IUFD This study aims to determined the risk factors of low birth weight in hyperthyroidism pregnancy at Magelang 2014.. This type of research is analytic observational case control approach, supplemented with a qualitative assessment. The sample were 41 pregnant womens who had low birth weight babies and 41 normal birth weightin 2014 and never take the test in the laboratory.Sampling technique Proportionate Stratified Random Sampling.Data analysis was performed using univariate, bivariate, multivariate, and qualitative analysis. Factors that are related of BBLR according to bivariate analysis are LILA (p=0,023; OR= 0,695; 95% CI=0,263-1,833) , increase in weight (p=0,010; OR=0,078; 95%CI=0,009-0,638), and anemia status (p=0,000; OR=0,126; 95% CI=0,046-0,344), while according to multivariate analysis are hipertiroidism in pregnancy, increase in weight and anemia status. The conclusion of this research is a women pregnancy had a hypertiroidsm status, increase in weight (<6kg atau >12 kg) and anemia status (Hb<11gr%) have the probability of having a baby with LBW is 89%.
